Menopause is generally defined as 12 months without menstruation. New red, pink or brown bleeding after that point—including a single episode—should be assessed. Many possible causes are benign, but evaluation is important because postmenopausal bleeding can also be an early sign of changes in the endometrium.

## What an assessment may include

1. Review of the bleeding, medical history, medicines and hormone treatments.
2. Gynecologic examination to identify the source of bleeding.
3. Transvaginal ultrasound when appropriate.
4. Endometrial sampling based on findings and individual risk.
5. Hysteroscopy when direct examination of the uterine cavity or directed treatment is needed.

Not every patient needs every study. Testing and treatment depend on the history, examination and findings.

## Seek care

Do not wait for bleeding to happen again before requesting an assessment. For severe bleeding, fainting, severe pain or another urgent symptom, seek urgent medical care rather than relying on website content.
